If you have insurance, when we get all the information below we will ask additional questions (for example: subscriber to policy name, DOB subscriber, patient name, patient DOB, your address, insurance company name, insurance ID#, group #, # to insurance company contact listed on the back of your card, and what times you are able to come into the office).

 Business owner SLP Kelly currently handles all administrative work as well as seeing patients, however is working on adding administrative staff in the future (which is something Fusion Therapies is super excited to watch grow!)  Feel free to ask questions along the way.
